Several times. You should see Final Order. He impales Cloud through the chest TWICE and Cloud still fights him off! Granted, he snuck up on Sephiroth and cut halfway through him with one stab of a ridiculously long sword but Sephiroth is SEPHIROTH and therefore is immune to almost dying.
